Plate for the reconstruction of the tibial plateau in prosthetic replacement - RK-PLATE

Fixation plate for tibial graft, with application in reconstruction of the tibial plateau in the presence of a prosthetic replacement.

Description

The RK-PLATE has been designed by the Department of Mechanical Engineering of the Instituto Tecnológico de Canarias in collaboration with Dr. Guillermo de la Barreda López (Orthopedic Service of the University Hospital of the Canary Islands).
The life expectancy growth has increased the number of prosthetic reviews in the knee joint as well as its pathologies that implies a mass bone loss mainly at the tibial level, which constitutes a serious surgical problem in the moment of the prosthetic replacement, which must be substituted by bone graft or implantable spacers incorporated within the prosthetic.
This possibility has two disadvantages: firstly, the need to increase of prosthetic modularity, associated with greater rates of loosening and on the other hand, the metallic substitutes involve an additional sacrifice of bone stock which is not recommended, especially if the patient has a life expectancy greater than the half-life of the prosthetic.
With the intention of regenerating the lack of bone, a containment plate has been developed using fragmented bone (bone chips) for reconstructing the tibial plateau in presence of a prosthetic component. In uncontained bone defects, the plate provides to the graft the necessary containment for maintaining its placement and the sufficient stability so that it can receive moderate stimuli of mechanical compression which are indispensable for the consolidation.
The RK-PLATE is a fixing plate, manufactured in stainless steel AISI 316LVM, for the tibial graft with application in reconstruction of the tibial plateau in the presence of a prosthetic replacement, both the left limb like the right limb. The plate is pre-bent in a standard shape and the surgeon will carry out the final folding with an optimal adjustment on patient's tibia (moldable plate to the patient's anatomical features) in the operating room. It is achieved combining the thick of the plate with a holes pattern which allows the adequate vascularisation of the area, with the purpose of favouring the rapid osteointegration of the bone graft. It has two holes for the insertion of the locking screws (cortical screws) which are oriented in such manner that avoid the prosthetic stem. The plate acquires a good final adjustment through proximal cerclages if the case so required or the use of unicortical screws.

Potential Benefits and Uses


The fundamental feature of the RK-PLATE, manufactured in stainless steel in two versions, left and right, is the bone graft containment, providing a high mechanical strength and favouring an adequate re-vascularisation of the graft. In addition, the plate is pre-bent in a standard shape, easily mouldable to the different anatomies and compatible with the tibial implantation with stem. It allows the bicortical fixation.

Transfer / Commercialization

The design has been verified through mechanical tests, but the process of certification is still pending (CE marked).
