Surgical needle holder for suturing areas with difficult access - PORTA

Device which makes it possible to make suture stitches in very restricted and deep surgical fields.

Description

The needle holder, PORTA, has been designed by the Department of Mechanical Engineering of the Instituto Tecnológico de Canarias in collaboration with Dr. Ignacio Díaz de Tuesta.
The current manual suture systems (needle holders) present a number of drawbacks which intend to be solved with the design of a new device. First, the traditional needle holders lose efficiency and accuracy when the plane of the needle comes close to the instrument. Secondly, the difficulty for reaching inaccessible suture areas should be solved with the surgeon's ability for moving the needles in planes that are not perpendicular to the axis of the needle holder. The most unfavourable case consists of suturing the tissues that are in the bottom of the surgical field and in a perpendicular plane respect to the surgical approach.
The device enables the surgeon suturing in restricted and remote surgical areas as the case may be the suture of an aorta prosthetic. A head of reduced size grabs and releases the needle, providing circular movements in both directions that allow the entry and exit of the needle into the tissue to be sutured. The working space required is reduced, and for this reason, this device makes possible to minimise the surgical fields in several branches of surgery.
This device is manufactured in titanium alloy Ti6Al4V and allows handling needles and move them in any plane parallel to the long axis of the instrument, allowing three degrees of freedom controlled by finger movements, consisting of: rotation of the head which contains the clamps which hold the needle, opening and closing of the clamps which hold the needle and the selection of the direction of spindle rotation (direction of needle rotation).

In addition, the device uses the resources that are familiar to the surgeon (lever arm to multiply the strength exerted by the thumb on the needle, locking mechanism) to avoid if possible a specific learning curve of the device.

Potential Benefits and Uses

This device, named PORTA, enables suturing in restricted and deep surgical areas, presents an optimal solution in terms of performance versus cost when compared with the traditional needle holders and the robotic systems. Its handling is similar to the conventional needle holder and, in addition, allows three grades of freedom handled by the fingers movements: the opening and closing of the clamps which hold the needle, the rotation and the selection of the direction of spindle rotation (direction of needle rotation).
