Plate for the proximal surgery of child and adolescent femur- HUMIC®

Adaptation of a nail-plate for the hip that optimizes the implantation technique and improves fixation of the osteotomy.

Description

The HUMIC® plate (Hospital Universitario Materno Infantil de Canarias), designed by the Department of Mechanical Engineering of the Instituto Tecnológico de Canarias in collaboration with Dr. Enrique Recarte, arises from the need to replace the obsolete osteosynthesis methods of hip osteotomies in children and adolescents. Its use is recommended in the following pathologies: congenital hip dysplasia, paralytic hip luxation, coxa valga or vara, fractures and Legg-Calvé-Perthes disease.
The HUMIC® system is an adaptation of a nail-plate for the hip which optimises the implantation technique and improvement of the osteotomy fixation providing to the cut plane of the bone only mechanical axial compressive stimuli.
The system consists of two surgical sets: one paediatric (children of 1 to 5 years) and another adolescent. Each set consists of two trays, one of implants and the other of the specific surgical instruments to carry out the surgical technique of implantation.
The plate to be implanted is made of stainless steel 316 LVM (ISO 5832-1) and consists of a cephalic no telescopic screw, a plate, two anti-rolling proximal wings and self-compression distal holes which are inserted the cortical screws that provide the self-compression of the bone fragments. The cephalic screw of the selected length is placed in the proximal fragment of the osteotomy in the direction of the axis of the femur neck. The chosen plate (available in 110º and 135º) is moulded and fixed to the bone (proximal fragment bone by two unicortical screws and distal fragment bone by bicortical screws which fix it on the lateral face of the femur.)
The surgical instruments consist of instrumentation for the preparation of the proximal fragment of the bone, the folding and positioning of the plate, placement of the cephalic screw and the rescue of the implanted material with a consolidated cut, 6 - 12 months after implantation.
In order to facilitate the surgical operation, a software application of three-dimensional simulation of osteotomies (OSTEOTOMIC) has been developed from the patient anatomical details. This software helps the surgeon at the phase of surgical planning, making it possible for surgeon to choose the optimum location and orientation for the cuts. OSTEOTOMIC is a scalable software which has a simple user interface, with 3D navigation and is easily learned by the surgeon.
OSTEOTOMIC visualises different 3D formats and has its own descriptor in XML of the virtual scene to be reproduced.
The software, in an interactive manner, makes it possible to carry out osteotomies to correct different pathologies of the femur. The current version has been clinically validated and may be used for simple planning prior to the operation, for the training of medical residents or alternatively as a demonstration of the surgical process for interested persons (relatives of the patient, students, etc.).

Potential Benefits and Uses

The HUMIC® plate presents a number of competitive advantages with regard to obsolete products or techniques in use. Among these is the bending in situ of the implant, which permits a perfect adjustment between plate and bone; the stable fixing of the proximal fragment by means of the placement of unicortical screws in the proximal wings of the plate; specialisation of the implant for osteotomies thanks to a non-telescopic cephalic screw; and, finally, it presents an easy approach of the distal fragment and a good compression on the cutting plane.

Transfer / Commercialization

En los últimos años, la comercialización de la placa HUMIC se llevó a cabo por la empresa MBA como distribuidor exclusivo para España, Italia y Portugal, países en los que ya se han implantado más de 200 placas. A partir del año 2010, el Instituto Tecnológico de Canarias ha cedido una licencia de uso a la empresa Osteofarma para la comercialización de esta tecnología. La demanda de este producto es inmediata y su disponibilidad también, ya que se dispone de stock de material para servir. La propiedad industrial de la placa está protegida a través de la patente española con nº 200302202(3) y de la patente internacional PCT con nº ES03/00599.
